    IMPORTANT ANNOUNCEMENT REGARDING
    MEDICAL TREATMENT
    for
    ON-THE-JOB INJURIES

This is to notify all CU-Boulder employees of an important change regarding
medical treatment for on-the-job injuries under the CU workers' compensation
program. Please be sure and share this information with those employees in
your department who do not have e-mail access.

The Designated Medical Provider (DMP) for on-the-job injuries is changing.
Effective June 1, 2007, Wardenburg Health Center will begin to phase out as the
DMP and the transition to the acting DMP is to be completed by July 1, 2007.

This means that beginning June 1, 2007, any new work-related injuries will be
treated at Arbor Occupational Medicine, located at 4790 Table Mesa Drive, Suite
200, Boulder, CO 80303. They can be contacted at (303) 443-0496 between the
    hours of 8-5 p.m.

As always, call 911 for life or limb threatening emergencies, and then contact
Arbor and University Risk Management for further instructions.

Employees who are currently being treated at Wardenburg, will be transitioned
to Arbor during the month of June in coordination with their Wardenburg
    physician.

University Risk Management is currently in the process of working with
Procurement Services Center on an RFP to choose permanent Designated Medical
Providers, as required by new legislation. It is anticipated that the
providers will be chosen within eight weeks from now.

We will keep you up to date on new information as soon as it becomes available
via electronic listserves and our website. Further information, including a
map to Arbor, can be found at:
