University Supervisor Resources
Thank you for your willingness to supervise our student teachers this semester. We hope that the experience will be rewarding for both you and the student teacher. In order to provide you with some guidance and background, we have made the following materials available:
University Supervisor's Handbook
This handbook contains useful information, suggestions, and activities to facilitate the student teacher-university supervisor relationship. Please read carefully before you begin to work with your student teacher .
Cooperating Teacher’s Handbook
This handbook contains information to facilitate the student teacher-cooperating teacher relationship. This handbook is included in your packet so that you are familiar with the information and instructions the cooperating teacher has received .
Student Teacher’s Handbook
This handbook contains information to facilitate the student teacher’s experience. This handbook is included in your packet so that you are familiar with the information and instructions that the student teacher has received.
Planning Calendar
This calendar contains noteworthy dates to help you keep your student teachers informed about workshops and meetings; a suggested time line for you to follow for observations; and deadlines for the completion of materials.
Observation Records (Required)
Three different formats are available (Form A (.pdf) (.doc), Form B (.pdf) (.doc), Form C (.pdf) (.doc)). These forms must be completed during the classroom observations of your student teacher and submitted to the Office of Student Services immediately after each observation. A Guide has also been included to assist you in understanding and completing these forms.
Mid-Assignment Assessment (Required) (.pdf) (.doc)
This assessment must be completed halfway through the student teacher’s placement in collaboration with the cooperating teacher and your student teacher. A Guide has also been included to assist you in understanding and completing this form.
Mid-Assignment Growth Plan (Required) (.pdf) (.doc)
This growth plan must be completed in conjunction with the Mid-Assignment Assessment and in collaboration with the cooperating teacher and student teacher .
Final Student Teacher Assessment (Required) (.pdf) (.doc)
The final evaluation must be typed on the final evaluation form and may include a letter of recommendation on school letterhead. Please submit to the Office of Student Services by the 3rd Friday in December (Fall semester) or the 3rd Friday in May (Spring semester).
Source of Evidence Checklist (Required)
The source of evidence checklist must be completed by student teacher candidates showing evidence that they have completed each of the state standards. This checklist must be verified by the cooperating teacher and the university supervisor.
Teacher Work Sample Proficiency Verification Form (Required)
This verification form is completed by the university supervisor and cooperating teacher to show that the student teacher has successfully completed each of the Teacher Work Sample elements.
The Teacher Work Sample Resource Guide provides detailed information about preparing your work sample.
Student Teacher’s In-School Schedule (Required)
Student teachers will use this form to provide you with important information about their teaching day. You can use this to plan your visits. Should a deviation from the schedule occur (e.g. an assembly, teacher inservice day, etc.) the student teacher should inform you of the change .
University Supervisor Evaluation of Cooperating Teacher (Required )
An on-line assessment link will be sent by email near the end of the semester.
How Do You See Me? (Optional)
Student teachers may use this or a similar assessment to receive feedback from the students with whom they work.
