University Supervisor Resources

Thank you for your willingness to supervise our student teachers this semester. We hope that the experience will be rewarding for both you and the student teacher. In order to provide you with some guidance and background, we have made the following materials available:

Mentoring Your Student Teacher
This handbook outlines the role and responsibilities for the student teaching semester. This handbook contains information, suggestions, and activities to facilitate the student teacher, cooperating teacher, and university supervisor triad. Please read this handbook and the Professional Practice in Student Teaching (Student Teacher Handbook) in preparation of the student teaching semester.

Professional Practice in Student Teaching (Student Teaching Handbook)
This handbook outlines the student teacher's role and responsibilities for the student teaching semester. This handbook contains information to facilitate the student teacher's experience and should be read by the student teacher, cooperating teacher, and university supervisor.

Student Teacher's In-School Schedule (Required)
The student teacher may copy the cooperating teacher's in-school daily/weekly schedule or use this form for the benefit of both the ST and the University Supervisor. The in-school schedule will provide the supervisor with important information about the teaching day. Supervisors use this to plan visits—should a deviation from this schedule occur (e.g., an assembly, teacher in-service day, etc.), the ST should notify the supervisor.

Getting Ready for the Semester (Optional)
This extended set of checklists is for student teachers to use to become familiar with their assigned school and prepare for classroom teaching. This can be used as a resource throughout the semester.

The Student Teacher Plan for Teaching (Optional)
This plan will help the student teacher organize their semester. The student teacher should consult with the cooperating teacher when filling in this semester plan for teaching. This plan may be shared with the supervisor.

Weekly Student Teaching Activity Plan (Optional)
This form, if used, is completed each week by the student teacher in consultation with the cooperating teacher and shared with the university supervisor (optional).

Observation Records (Required)
Three different formats are available (Form A (.pdf) (.doc), Form B (.pdf) (.doc), Form C (.pdf) (.doc) for use by the university supervisor (required) and cooperating teacher (optional).

Quality Responsive Classroom Form (QRC)
The QRC is an alternate program-approved observation protocol that focuses on student and teacher actions/interactions as well as classroom climate. Supervisors who have completed a QRC training session may use this observation tool for student teaching observations.

Student Teaching Standards Verification Form (Required ) (.doc)
This form is a place to record progress toward and eventual verification of satisfying Performance-Based Standards for Colorado Teachers. This form should be kept by and up-dated on a regular basis by the CT and ST. The US may want to add to it during observations. At the mid-assignment/assessment/growth plan conference, the supervisor, cooperating teacher, and student teacher collaboratively document progress toward satisfying standards and set growth plan goals. At the final conference, the supervisor and student teacher ensure all standards have been satisfied. All areas must receive a rating of 3 by the final assessment in order for the student teacher to be recommended for licensure. The supervisor submits a completed form to the Office of Student Services. This form is retained in the student teacher's permanent file.

It is the responsibility of the CT and ST to keep this form and update it on a regular basis. The US may add to it when the supervisor observes.

edTPA Making Good Choices
This document can help you and your student teacher understand the purpose of each edTPA task. Making Good Choices will be emailed directly to you.

edTPA Submission Guidelines for DVD
These guidelines will be emailed to the student teachers. They are required to submit a DVD of their edTPA and supporting documents to the supervisor.

edTPA Video Materials Permission Form English and Spanish Versions
Student teachers are asked to use this official video permission form to secure parent/guardian permission to record video and gather artifacts of student work for their edTPA. For legal purposes, these permission forms are to be used instead of ones collected by the school. If these forms are not found on the website, they will be emailed directly to you.

Final Student Teacher Assessment (Required) (.pdf) (.doc)
Please complete the final evaluation form and/or letter of recommendation. You may include your evaluation on the letter of recommendation. If you write a letter of recommendation instead of using the final evaluation form, use university letterhead. If you send a letter, please include the final evaluation form with information at top and signatures at the bottom of form. You will need to collect the cooperating teacher's evaluation form or letter of recommendation and turn them in to the Office of Student Services.

University Supervisor Evaluation of Cooperating Teacher (Required)
An on-line assessment link will be sent by email near the end of the semester (1st Monday in December for fall placements and 1st Monday in May for spring placements). The authors of evaluations will remain anonymous.
