Student Teacher Resources

Below, please find information and forms pertaining to your student teaching placement. We recommend that you review all the documents and read the student teaching handbook carefully, since many of your concerns or questions may be addressed.

Student Teacher Handbook
This is a guide outlining roles and expectations for student teachers. This handbook includes general information on student teaching, the university’s expectations of you, and advice that should be useful during your student teaching experience.

Student Teaching Syllabus
This syllabus will provide you with an overview of the requirements and expectations for the student teaching semester.

Student Teacher’s In-School Schedule (Required)
This form is for the benefit of both you and your University Supervisor. Use the form to provide your supervisor with important information about your teaching day. Supervisors use this to plan visits—should a deviation from this schedule occur (e.g., an assembly, teacher in-service day, etc.), you should notify your supervisor.

Getting Ready for the Semester
This extended set of checklists is for you to use to become familiar with your assigned school and prepare for your classroom teaching. Use this as a resource throughout the semester.

Observation Records (Required)
Three different formats are available (Form A (.pdf) (.doc), Form B (.pdf) (.doc), Form C (.pdf) (.doc) for use by your University Supervisor (required) and your Cooperating Teacher (optional). A Guide has also been included to assist you in understanding the observation process and criteria.

Mid-Assignment Assessment (Required) (.pdf) (.doc)
Halfway through your placement, you, your Cooperating Teacher and your University Supervisor will spend time assessing where you’ve been and goals for the remainder of your assignment. This form is intended for you, your Cooperating Teacher and your University Supervisor to collaboratively reflect on your growth as part of that process. A Guide has also been included to assist you in understanding and completing this form.

Mid-Assignment Growth Plan (Required) (.pdf) (.doc)
After you have completed the Mid-Assignment Assessment, This growth plan must be completed in conjunction with the Mid-Assignment Assessment and in collaboration with the cooperating teacher and student teacher.

Final Student Teacher Assessment (Required) (.pdf) (.doc)
Both your Cooperating Teacher and your University Supervisor will be completing this form. The original will be sent to Career Services for your placement file, and the Office of Student Services will keep a copy.

Source of Evidence Checklist (Required)
The source of evidence checklist must be completed by student teacher candidates showing evidence that they have completed each of the state standards. This checklist must be verified by the cooperating teacher and the university supervisor.

Teacher Work Sample Proficiency Verification Form (Required)
This verification form is completed by the university supervisor and cooperating teacher to show that the student teacher has successfully completed each of the Teacher Work Sample elements.

The Teacher Work Sample Resource Guide provides detailed information about preparing your work sample.

Student Teacher Evaluation of Cooperating Teacher (Required)
We are interested in obtaining feedback from you about the effectiveness of your Cooperating Teacher. An on-line assessment link will be sent by email near the end of the semester.

Student Teacher Evaluation of University Supervisor (Required)
We are also interested in obtaining feedback from you about the effectiveness of your supervisor. An on-line assessment link will be sent by email near the end of the semester. After grades are submitted, overall results will be shared with the supervisors.

How Do You See Me? (Optional)
One of the best ways to assess your progress and success is to ask your students for feedback. This form may be given to the students you teach. We offer it as an option, not a requirement, but we encourage you to use this or something like it to assist you in self-reflection at some point during your assignment.
