Cooperating Teacher Resources
Each student teacher is assigned a University Supervisor to monitor the student's progress and act as a liaison between the student teacher, you, and the University. In order to provide you with some guidance and background, please review the materials listed below. Additional roles and responsibilities are outlined in the Cooperating Teacher Handbook. Please peruse them, and feel free to call the Field Experiences office at 303-735-1908 if you have questions or concerns about what you see here.
Cooperating Teacher and University Supervisor's Handbook
The Cooperating Teacher and University Supervisor's Handbook outlines the role and responsibilities for the student teaching semester. This handbook contains information, suggestions, and activities to facilitate the student teacher, cooperating teacher, and university supervisor triad. Please read this handbook and the Student Teacher Handbook in preparation of the student teaching semester.
Student Teacher’s Handbook
The Student Teacher Handbook outlines the student teacher's role and responsibilities for the student teaching semester. This handbook contains information to facilitate the student teacher’s experience and should be read by the student teacher, cooperating teacher, and university supervisor.
Supervision of Student Teacher Log
The cooperating teacher should log one-on-one contact hours with student teacher. These hours may be used for up to 6 hours of renewal credit. The cooperating teacher is responsible for completing this log and submitting required paperwork for licensure renewal.
Cooperating Teacher Profile
The cooperating teacher profile should be completed by cooperating teacher and given to the student teacher.

Although not required for cooperating teachers, these forms may be used during or after you make observations of your student teacher. A Guide has also been included to assist you in understanding and completing these forms.
Quality Urban Classroom Form
The QUC is an alternate program-approved observation protocol that focuses on student and teacher actions/interactions as well as classroom climate. Supervisors who have completed a QUC training session may use this observation tool for student teaching observations.
Student Teaching Standards Verification Form (Required) (.pdf) (.docx)
This form is a place to record progress toward and eventual verification of satisfying Performance-Based Standards for Colorado Teachers. The US is responsible for maintaining the official copy of the Student Teaching Standards Verification Form. At the Mid-Assignment Assessment/Growth Plan Conference, the supervisor, cooperating teacher, and student teacher collaboratively document progress toward satisfying standards and set growth plan goals; following the Mid-Assignment conference, the supervisor makes copies for the CT, ST, and one for the student teacher’s file. At the Final Conference, the supervisor and student teacher ensure all standards have been satisfied, and then the supervisor submits a completed form to the Office of Student Services. This form is retained in the student teacher’s permanent file.
Final Student Teacher Assessment (Required) (.pdf) (.docx)
Please complete the final evaluation form and/or letter of recommendation. You may include your evaluation on the letter of recommendation using school/district letterhead. If you send a letter, please include the final evaluation form with information at top and signatures at the bottom of form The final evaluation form and letter should be submitted to the Office of Student Services by the 3rd Friday in December (Fall semester) or the 3rd Friday in May (Spring semester).
The Teacher Work Sample Outline and Rubric provides detailed information about preparing your work sample. This form is only used by music education students.
Teacher Work Sample Proficiency Verification Form (Required)
This verification form is completed by the university supervisor and cooperating teacher to show that the student teacher has successfully completed each of the Teacher Work Sample elements. This form is only used by music education students.
Cooperating Teacher Evaluation of University Supervisor (Required )
An on-line assessment link will be sent by email at the end of the semester (1st Monday in December for fall placements and 1st Monday in May for spring placements).
EDUC 4691 - 4732 Student Teaching Syllabus
This syllabus will provide you with an overview of requirements and expectations for the student teacher during the student teaching semester
Weekly Student Teaching Activity Plan(Optional)
This form, if used, is completed each week by the student teacher in consultation with the cooperating teacher and shared with the university supervisor.
