Cooperating Teacher Resources

Each student teacher is assigned a University Supervisor to monitor the student's progress and act as a liaison between the student teacher, you, and the University.  In order to provide you with some guidance and background, please review the materials listed below.  Additional roles and responsibilities are outlined in the Cooperating Teacher Handbook.  Please peruse them, and feel free to call the Field Experiences office at 303-735-1908 if you have questions or concerns about what you see here.

Introduction Letter

Cooperating Teacher Roles and Responsibilities
This letter outlines the relationship of a cooperating teacher to a CU Boulder student teacher, and our expectations over the course of the semester.

Cooperating Teacher's Handbook
This handbook contains useful information, suggestions, and activities to facilitate the student teacher-cooperating teacher relationship.

Cooperating Teacher Assessment of University Supervisor (Required)
An on-line assessment link will be sent by email near the end of the semester.

Mid-Assignment Assessment (Required) (.pdf) (.doc) and Guide to the Mid-Assessment (Required)
This assessment will be completed halfway through your student teacher's placement in collaboration with the university supervisor and your student teacher.  A Guide has also been included to assist you in understanding and completing this form.

Mid-Assignment Growth Plan (Required) (.pdf) (.doc)
This growth plan will be completed in conjunction with the Mid-Assignment Assessment in collaboration with the university supervisor and student teacher.

Final Student Teacher Assessment (Required) (.pdf) (.doc)
The final evaluation must be typed on the final evaluation form and/or a letter of recommendation on school letterhead and submitted to the Office of Student Services by the 3rd Friday in December (Fall semester) or the 3rd Friday in May (Spring semester).

Source of Evidence Checklist (Required)
The source of evidence checklist must be completed by student teacher candidates showing evidence that they have completed each of the state standards. This checklist must be verified by the cooperating teacher and the university supervisor.

The Teacher Work Sample Outline and Rubric provides detailed information about preparing your work sample.

Teacher Work Sample Proficiency Verification Form (Required)
This verification form is completed by the university supervisor and cooperating teacher to show that the student teacher has successfully completed each of the Teacher Work Sample elements.

Cooperating Teacher Evaluation of University Supervisor (Required )
An on-line assessment link will be sent by email near the end of the semester.

EDUC 4691 - 4732 Student Teaching Syllabus
This syllabus will provide you with an overview of requirements and expectations for the student teacher during the student teaching semester

Weekly Student Teaching Activity Plan(Optional)
This form, if used, is completed each week by the student teacher in consultation with the cooperating teacher and given to the university supervisor.  The form should be signed by both the student teacher and the cooperating teacher.  Signatures by the student teacher and the cooperating teacher indicate that weekly cooperative reflection on the student teacher’s performance has taken place, and that a plan for action has been identified.  The student teacher is expected to act on suggestions for improvement which are provided on the completed form, including the plan of action.

Observation Records Form A (.pdf) (.doc), Form B (.pdf) (.doc), Form C (.pdf) (.doc) and Guide to Observations(Optional)
Although not required for cooperating teachers, these forms may be used during or after you make observations of your student teacher. A Guide has also been included to assist you in understanding and completing these forms.
