Anseth Lab Equipment Permission Form for Outside Users

Introduction: While we welcome and encourage the use of our equipment, the Anseth lab has a strict policy that all users must be adequately and sufficiently trained on equipment to insure that all lab equipment is used correctly.  This is primarily meant to minimize problems associated with equipment misuse and high repair costs.  We appreciate your cooperation in this process.

Instructions: Every outside user must complete this form and obtain permission from either Professor Anseth (Kristi.Anseth@colorado.edu), Victoria Bingham (Victoria.Bingham@colorado.edu) or Pete Mariner  (Peter.Mariner@colorado.edu).  Approval is given for a requested time frame.  Since protocals may change, training and permission forms should be updated if you plan to use equipment for more than one year.  This form should be completed and turned into Victoria or Pete prior to equipment usage.  The type of equipment available and individuals responsible for training on each piece of equipment can be found on our group website  (www.colorado.edu/che/ansethgroup).
