How do I fill out a CCC pledge card?
- NEW: You can now pledge online through a secure site created just for State of Colorado employees! Log onto the site using your CU email address as your Username. For your password, please enter your Employee ID number followed by the capitalized first two letters of your last name. All pledges will receive an email confirmation.
- Use the pledge card provided with the CCC material, or print out
the blank CCC
pledge card.
- The form is divided into two sections: CCC Office Copy, and the Payroll
Copy.
- Begin by filling out the CCC Office Copy. Make sure the following
areas are filled out:
- Your Name
- Your Campus Box
- Your Department at CU-Boulder
- Put your Employee ID# on the location code line
- Check whether you are contributing through payroll deduction
or one-time donation
- Indicate the amount you are contributing
- Indicate the agencies (by code number)
you are donating to (read the CCC brochure online.)
- If you wish to donate via a one-time charge to your credit card, fill
out the credit card section below the payroll deduction section.
- Fill out your personal information if you do not wish to remain anonymous, and are interested in receiving
recognition from the organizations for your contribution
- If you are donating through payroll deduction, please fill out the
Payroll Copy. Include the following:
- Your Name
- Your Campus Box
- Your Department at CU-Boulder
- The amount, number of months and total for the deduction
- Sign the form and date it.
- You do not need to put your Employee ID number on the form.
- If you are donating by check, please make all checks payable to the
Colorado Combined Campaign.
- Make a copy of the pledge card if you plan to claim a charitable tax deduction on your federal income taxes. This copy can serve
as your proof of donation for your tax records.
- Mail the pledge card and your check,
if applicable, to Campus Box 584, attention:
CCC. There is a yellow envelope included in
your pledge packet for this purpose.
