• Approved for the accommodation.
• Submit a list of books or other materials; including:
• complete course number
• instructor name
• complete title of text
• author or authors (editor if applicable)
• edition or publishing date
• ISBN number (if possible)
• Required to purchase each book requested
• May need to provide proof of book ownership
• May need to provide hardcopy
• May need to provide syllabus
