Colorado State Law requires new freshman, transfer, and graduate students to submit proof of two doses each of measles (rubeola), two rubella (German measles), and two mumps (or “MMR”) vaccinations. This is a mandatory requirement for all college students born on or after January 1, 1957. The university requires all incoming international students and students with risk factors for Tuberculosis to be tested for TB. If any student is positive for any of the risk factors, they must be tested for TB. The mandatory Tuberculosis Information Form is a screening for risk factors. While menengitis vaccination is not required, students are highly encouraged to get the vaccine. Students are required to fill out a form either showing proof of the vaccination or waiving it.
Submit forms by fax, mail, or in person by the end of August. Visit http://www.colorado.edu/healthcenter/forms/required-immunization-forms for more information.
