Disclosure Form (DEPA)

To access the online DEPA form:

1. Log in to CUConnect at https://cuconnect.colorado.edu. (You will need your CU Login name and IdentiKey password.)
2. Click on the Academics & Research tab (left side of the page next to the Welcome! tab).
3. The DEPA link should be on the lower right side under Faculty Reporting.

   Please note  CUConnect is being replaced by MyCUInfo during 2011-2012. https://portal.prod.cu.edu/MyCUInfoFedAuthLogin.html

   
   Once  MYCUInfo is in place, the process will be as follows:

   1. Click on top tab: CU Resources
   2. Click on Faculty Reporting (LOWER left side in the shaded area, not under the simple Reporting tab at top left side)
   3. Scroll down until the DEPA link become visible
