Benefit Applications and Other Forms




Forms for students who have never applied for their benefits

22-1990 - Application for VA Education Benefits If you've never used your VA educational benefits before, you'll need to fill this form out and return it to our office. This is the initial benefit claim application that starts your benefits for the first time. This applies only to GI Bill Chapters 30, 32, 1606, and 1607 students. Chapter 30 and 32 applications must be accompanied by a certified copy of member 4 of your DD214. Chapter 1606 applications must be accompanied by Notice of Basic Eligibility which a student obtains from their Reserve or National Guard unit.
22-5490 Application for Survivor's and Dependant's Educational Assistance If you've never used your VA educational benefits before, you'll need to fill this form out and return it to our office. This is the initial benefit claim application that starts your benefits for the first time. This applies only to Survivors or Dependents of 100% disabled or deceased veterans Chapter 35
Spring/Fall Course Load Summary Form Course Load Summary Forms are required for every period of enrollment in which you would like to receive your VA educational benefits. You will not have your enrollment certified with the VA (i.e.- you will not receive a payment) unless this forms is turned in to Veterans Educational Benefits. Please ensure that you complete the topmost box indicating which term you are requesting to be certified. **Good Rule of Thumb- complete a Course Load Summary Form each time you enroll in or withdraw from courses.**

Forms for students who have applied for or received benefits previously, but have changed schools or major, have taken a break in their enrollment or wish to apply for a different benefit

22-1995 Request for Change of Program or Place of Training This is the form you must fill out to request a change of program or place of training with the VA. This form applies to transfer students and others that have used VA educational benefits in the past and changed schools, changed majors or taken breaks in their education. This applies only to GI Bill Chapters 30, 32, 1606, and 1607 students.
22-5495 Request for change of Program or Place of Training for Survivors' and Dependants' Educational Assistance If you are a Chapter 35 Survivor or Dependent receiving VA Educational Benefits, you must complete this form to request a change of program or place of training with the VA. This form applies to transfer students and others that have changed schools or taken breaks in their education. This applies only to Chapter 35 students.
Spring/Fall Course Load Summary Form Course Load Summary Forms are required for every period of enrollment in which you would like to receive your VA educational benefits. You will not have your enrollment certified with the VA (i.e.- you will not receive a payment) unless this forms is turned in to Veterans Educational Benefits. Other Forms

Advance Payment Request If you are applying for VA benefits for the first time, or you are returning to school after more than 30 days break between periods of enrollment, and you are planning to be enrolled at least half-time, you are eligible for advanced payment, (excluding Active Duty and Vocational Rehabilitation recipients). You must submit a request for advance payment, along with a course load summary form and application (if needed), to the Veterans Educational Benefits Office. You should apply at least 60 days prior to the start of the semester in which you plan to attend to allow the Veterans Administration sufficient time to process your request, and Advance Pay Requests will not be accepted within 30 days of the start of classes. Advance pay is a great way to offset initial costs at the beginning of a term. However, keep in mind that an advance payment includes the first partial month and the first full month of benefits. This means you will have to wait approximately two months before you will receive another payment from the VA. (Example: for Fall advance payment, you will be paid for August 28 to August 31 and all of September; and the next time you will receive a monthly payment is not until you verify your enrollment for the month of October).
Course Repeat Form VA regulation prohibits students who receive VA educational benefits from having these benefits pay for the same class more than once unless required for the student’s degree program. In order to verify that a course you are repeating is required to satisfactorily complete your degree program, the Repeat form must be signed by your Academic Advisor before we will certify you for a course that you have already taken.
Graduation Letter/Termination of Benefits Form This form is used to terminate benefits upon graduation or withdraw from school. If transferring to another school it is important to complete this form in order to receive benefits at your new school.
