Heads, System Administration FROM: Meg Rowland, FCQ coordinator, 2-7069 SENDER: meg@stripe.colorado.edu DATE: October 5, 2000 SUBJECT: Faculty Course Questionnaires, Fall 2000 Your department office has just received its FCQ course list for fall term - this is your department's FCQ "order form," and it is due back to us by October 18, when we begin printing. If you are teaching a course with *multiple instructors*, please be sure to communicate with your department's FCQ coordinator so that everyone who wants an FCQ packet is on this list. Also, if an FCQ packet is needed early (before November 28), let your coordinator know - "early out" packets need to be ordered at least 2 weeks in advance for us to get them printed and delivered to you. If you would like to request optional questions on side B of your FCQs, go to our website at http://www.colorado.edu/pba/fcq. Scroll down and click on "requesting optional questions." If you don't want to use the web, your department FCQ coordinator has hardcopy forms you can use. The deadline for optional question requests is October 15. FCQs will arrive in your department office on or about November 27. If possible, look over your packet carefully at that time. If there are problems, we can reprint up to November 30; after that, there may be a $5 charge to your department if a "special attention" packet is needed. FCQ administration week this term is December 4-8. Please note that neither instructors nor TAs for the course may be present at any time during administration of FCQs.
