Special Action (SAF) and Change of Record (COR) Forms
Special Action Form (SAF)
(This form is only available to the Student in the Department)
Once initial deadlines for dropping and adding courses have passed each semester, students are required to have a Special Action Form completed with appropriate approval signatures. Specific deadlines for dropping and adding each semester may be found on the Registrar's website or posted in the Academic Advising Center.
Adding a Course
Students may add courses themselves via the web until the "add deadline." After the initial deadline and until approximately 3 weeks into the semester, an instructor's approval is needed. The student picks up a Special Action Form in the department, gets the instructor's approval, and turns the SAF in at the Registrar's office in Regent Hall. The course is then added to the student's schedule.
After this deadline, students must petition the Assistant Dean's office in Woodbury Hall in order to late add a course. Late adds at this point are for extenuating circumstances only. The student picks up a Late Add Petition in either the Academic Advising Center (Woodbury 109), online at http://www.colorado.edu/ArtsSciences/aacforstudents/forms.html, or from the academic advisor, reads and completes the petition, gets the instructor's signature, and takes the Late Add Petition to the Academic Advising Center, Woodbury 109, for review. Academic Advising notifies the student via their CU e-mail whether or not the petition is approved. If approved, Academic Advising notifies the Registrar's office in Regent Hall and the course is added to the student's schedule.
Dropping a Course
Students may drop courses themselves via http://plus.colorado.edu/ until the "drop deadline." After the "drop deadline," Arts and Sciences students only may drop courses through the Registrar Forms Channel on CUConnect. The course is then dropped from the student's schedule and a grade of W (withdraw) is assigned.
After the drop deadline, students must petition the Assistant Dean's office in order to late drop a course. The student picks up a Late Drop Petition either in the Academic Advising Center (Woodbury 109), online at http://www.colorado.edu/ArtsSciences/aacforstudents/forms.html, or from the academic advisor, reads and completes the petition, gets the instructor's signature, and takes the form to the Academic Advising Center, Woodbury 109, for review. Academic Advising notifies the student via their CU e-mail whether or not the petition is approved. If approved, Academic Advising notifies the Registrar's office in Regent Hall; the course is dropped from the student's schedule and a grade of W is assigned.
Change of Record Form (COR)
(This form is available to the Instructor Only)
The Change of Record form is used to submit grade changes for Arts and Science students. Unlike the SAF, students are not allowed to be in possession of the COR form themselves. The student contacts the instructor; the instructor fills out the form and signs it, and then sends it to the Academic Advising Center for the Assistant Dean's review. The reason for requesting the change must be clearly stated. If approved, the COR is forwarded to Academic Records in the Registrar's Office for the change to be processed. CORs should never be given to the academic advisor. The student can check his or her schedule to learn of the decision.
