Notarized Oath Requirements
(for the College of Arts and Sciences, based upon Provost Avery's April 1, 2005 memo)
According to Colorado Statutes/TITLE 22 EDUCATION/TEACHERS/ARTICLE 61 TEACHER EMPLOYMENT/22-61-104: "Every person employed to teach in any state university, college, junior college, community college, or technical college in the state of Colorado, before entering upon or continuing the discharge of his duties, shall take the following oath or affirmation; except that no person employed to teach in a temporary capacity who is a citizen of a nation other than the United States shall be required to take such oath or affirmation."
A copy of the oath form is mailed along with the letter of offer to all incoming tenure-track, tenured, and full-time instructor faculty; and now is included as a part of all offer letter templates. The oath form alone is available on the web in several places, one of which is: http://www.colorado.edu/ArtsSciences/facultystaff/administration/templates.html.
The original, oath form is to be kept in various locations, depending upon the faculty member's title, as listed below:
Tenure-track and Tenured faculty (Non-citizens in this category are considered to be permanent employees and must complete a notarized oath form.)
- Original form: Sent to Dean's Office along with accepted letter of offer. Kept in Faculty Affairs.
- Copies: In department with a note on where the original is located (Faculty Affairs). To employee.
Full-time Instructors and Senior Instructors (Non-citizens must submit an oath form with the non-citizen box checked, but their forms need not be notarized.)
- Original form: Sent to Dean's Office along with accepted letter of offer. Kept in Faculty Affairs.
- Copies: In department with a note on where the original is located (Faculty Affairs). To employee.
All Other Non-Student Faculty Who Teach: Lecturers, Part-time Instructors, Summer Appointments, Adjoint, Adjunct, Visiting, Professional Exempt, etc. (Non-citizens must submit an oath form with the non-citizen box checked, but their forms need not be notarized.)
- Original form: Sent to Dean's Office along with accepted letter of offer. Kept in Dean's Office.
- Copies: In department with a note on where the original is located (Dean's Office).To employee.
Graduate Part-Time Instructors (GPTI) (Non-citizens must submit an oath form with the non-citizen box checked, but their forms need not be notarized.)
- Original form: Kept in department with letter of offer.
- Copies: To employee.
