The main conference registration fee of $275 includes the general sessions,
the keynote address, and the final lunch panel.
The registration form offers the chance to register for one or both days
of the preconference workshop in conjunction with the main conference at
a discounted rate. Please see the registration form for the applicable fee.
To register, complete the registration form online or fax or mail the form
October 1 October 14, 2005. If you are attending preconference
workshops, please indicate your workshop choice on the registration form.
Late registration fees will apply to forms postmarked after
October 14. Please make checks payable to the University of Colorado. Additional
information will be sent with your registration confirmation.
The registration link will be deactivated on November 1. If you register
after November 1, please fax the form and bring your check with you if you
are not paying by credit card. The University of Colorado does not accept
purchase orders. Final payment must be made before or at on-site registration.
If we have no record of payment at registration, we will require payment
at that time.
We cannot bill for registration fees.
Full refunds will be given if a written request for cancellation is received
by October 15. No refunds will be made after that date. Requests may be

Refund requests will not be accepted on the telephone.
